During your initial consultation at Crescent Dental, we will explain your diagnosis and treatment options. In some cases, treatment can be done on the same day as the consultation. However, if you have a complex medical history or treatment plan, your evaluation and treatment will be scheduled in separate appointments.
To provide our patients with the best treatment, we request you bring the following items to your initial visit to our practice:
Please arrive at least 15 minutes before the start of your scheduled appointment time to fill out necessary paperwork.
Please note: A parent or guardian must accompany patients under 18 for their consultation visit.
Please call our practice at 985-532-3480 to schedule an appointment. Once you make an appointment, that time will be reserved for you. If, for some reason, you are unable to keep your scheduled appointment time, please call us with 48 hour notice to reschedule.
We do our best to remain on schedule and minimize waiting. Due to the fact that our practice provides a range of dental treatments, and various procedures may lengthen the time needed to complete a procedure.
As a family dentist, we believe quality dentistry should be affordable. We work hard to provide patients with tailored cosmetic and general dentistry services that are fairly priced. During your initial consultation, we will review costs and help you select a payment method that meets your needs and makes your dental care affordable and accessible.
We accept cash, checks, Visa, Discover, MasterCard, and debit cards. Third-party financing, often at no or low interest for qualified applicants, is available through CareCredit. Please see their websites for details and to complete an application online.
After you receive treatment, we will send you a monthly statement. Most insurance companies respond within 4 to 6 weeks. If your statement does not reflect your insurance payment, please contact our office. After your insurance has paid, any remaining balance is your responsibility.
You are fully responsible for all fees charged by this office regardless of your insurance coverage.

Seeking a dentist within your network may not be the best choice for you. Although we are a non-network provider, that does not mean you cannot receive treatment in our dental office. Our fees are usually within the usual, customary and reasonable charges allowed by your insurance company. Please contact our office with any questions.
